Tools and Resources for AATF Members
Use the following forms and tools to help you conduct your public education programs and request supplies to support your efforts. Print, complete and send forms to: Community Affairs Department, Gift of Hope Organ & Tissue Donor Network, 425 Spring Lake Drive Itasca, IL 60143. Expense Reimbursement Form
Use this Form to recoup travel expenses incurred for participating in authorized Gift of Hope or Donate Life Illinois events.

Use this form to schedule a public education program and to request brochures, giveaways and other materials needed to make your event a success.
