Tools and Resources for AATF Members
Use the following forms and tools to help you conduct your public education programs and request supplies to support your efforts. Print, complete and send forms to: Community Affairs Department, Gift of Hope Organ & Tissue Donor Network, 425 Spring Lake Drive Itasca, IL 60143. You may also fax them to 630/758-2603 or send them via e-mail attachment.
Expense Reimbursement Form
Use this Form to recoup travel expenses incurred for participating in authorized Gift of Hope or Donate Life Illinois events.
Advocate Expense Reimbursement Policy:
Gift of Hope provides expense reimbursement to Advocates who submit a completed Expense Reimbursement Form and appropriate supporting documentation for approved outreach opportunities. Advocates who travel for an authorized Gift of Hope event are reimbursed for mileage, public transportation, tolls and parking fees. We are unable to process reimbursement requests if they are turned in more than 30 days after the date the expenses were incurred. All expenses must be associated with a scheduled program. The Expense Reimbursement Form must include original receipts. Please make copies of your receipts and a copy of the Expense Reimbursement Form for your records.
Materials and Volunteer Request Form
Use this form to schedule a public education program and to request brochures, giveaways and other materials needed to make your event a success.
Program Outcome Report
Use our online form to track and report your volunteer activities so we can measure your volunteer contributions.
Questions from the Public Form
Use this form to document questions you get from people, but can't answer, and to capture their contact information so Gift of Hope staff can follow up.
